Sonic Compilation, if we're including re-releases on the same console. There was also the arcade version and a Japanese cell phone version from a few years ago.

I cheated: it was apparently released for the Blackberry late '09. That covers it as far as Moby tells me.

A little late, but... Sonic Mega Collection Plus was also released for PC, so there's that one too.

Some Samsung phones, if I remember right, had Sonic 2 pre-installed.
(Off-topic, but that does remind me. Whatever happened to Sonic Unleashed for iOS?)

What about
Rayman 2? It probably has more re-releases than any other 3D platformer out there:
- PC
- Nintendo 64
- Dreamcast
- PlayStation (also re-released on PSN)
- PlayStation 2 (under the name of Rayman Revolution in some territories, also re-released as part of the 10th Anniversary Pack)
- Nintendo DS (as Rayman DS)
- Nintendo 3DS (to be released as Rayman 3D)
- iPhone
The PlayStation release featured less levels, extra characters and English dialogue, whereas the PlayStation 2 release featured a bunch of new stuff and the ability to choose between gibberish and English.

Labyrinth was playable and had cool music. I can think of at least 7 other Sonic games that didn't fair nearly so well, so, like, step off, aight?

Those are the official licensed ports. It can also be found in flash, java, DS, graphic calculators, cellphones and so on. With the source code released, the skies the limit.
